If you’ve been off sick for more than 7 days
To request a sick note you must for an appointment with the doctor to discuss the reason for being off and in order to get your sick (fit) note for work. A certificate cannot be issued without an appointment.
If you’ve been off sick for 7 days or less
If you’re off work sick for 7 days or less you do not require a sick (fit) note however you may be asked to provide a self-certification form. You can Click Here to complete online at the gov.uk website.
